Ask any athletic trainer how many physical forms they actually receive versus how many were collected at tryouts, and you'll get a pause. Not because they don't know — but because the number is uncomfortable.

At one mid-sized high school, the athletic trainer estimated that 50 to 75 forms per season never make it from the coach who collected them to the training office. Not rejected. Not incomplete. Just gone. Somewhere between the gym and the filing cabinet, they disappear.

This isn't a scandal. It's a system failure that's been normalized to the point where nobody tracks it.

How Forms Get Lost

The loss pattern is remarkably consistent across schools. A coach collects physical forms at the first day of tryouts — maybe 25 or 30 at once. The forms go into a bag, a folder, or a stack on the bleachers. The coach runs tryouts. After practice, the stack goes home in the car or stays in the coaching office.

The transfer to the athletic trainer happens when the coach remembers, which might be the next day or the next week. By that point, forms have shifted. Some are in the car. Some fell out of the folder. Some got mixed in with other paperwork. The coach delivers most of the stack, but "most" isn't "all" — and nobody has a checklist to reconcile the difference.

This cycle repeats across every sport, every season. Fall sports alone might have four or five coaches each collecting forms independently. Multiply the leakage across all of them and the volume of lost forms adds up fast.

Why Nobody Tracks It

The reason this problem stays invisible is that there's no system designed to catch it. The coach doesn't have a receipt showing which forms were collected. The trainer doesn't have an expected count to check against. The AD has a roster of athletes who should have forms on file, but comparing that roster against what's actually been received requires manual reconciliation that rarely happens until someone asks.

By the time the gap is noticed — if it's noticed at all — the season is underway. The response is usually to contact the families of athletes whose forms are missing and ask them to submit again. The families, who already turned in a form, are confused and frustrated. Some comply. Some don't. And the cycle repeats next season because the underlying process never changed.

The lost forms aren't tracked because the system was never designed to track them. Collection and review are treated as a single step ("get the physicals in"), but they're actually two separate steps with a gap between them that nobody owns.

The Compliance Exposure

A lost form isn't just an administrative inconvenience. It's a compliance gap.

An athlete whose form was collected but never delivered to the trainer might practice and compete for weeks — or an entire season — without anyone confirming they were cleared. The school technically "collected" the form. But collection without review is just storage, and storage doesn't constitute compliance.

If that athlete gets injured, the question won't be "did the family turn in a form?" It will be "did the school review and clear this athlete before they participated?" If the form is in a coach's trunk instead of the trainer's filing cabinet, the answer is no — regardless of the family's good faith effort to comply.

This exposure isn't theoretical. It's the predictable outcome of a process that depends on informal handoffs between people who have competing priorities.

Closing the Gap

The handoff problem exists because physical forms start their journey in the wrong place. When collection happens at the point of tryouts — on a field, in a gym, through a coach — every form has to travel from there to wherever the review happens. That journey is where forms get lost.

The fix is to move collection upstream. When parents upload forms directly — through a portal, an email submission, or any process that doesn't involve handing paper to a coach — the form arrives in the review queue without a handoff. No coaching bag. No car ride. No "I'll turn these in tomorrow." The form goes from the parent to the reviewer in one step.
